San Diego Childhood Asthma Initiative
The San Diego Childhood Asthma Initiative is a community collaboration with the goal of improving asthma management for children. The Initiative is funded by grants. There are three components to the Initiative:
Care Coordination for Families
For children under with asthma, parents receive care coordination from an Asthma Coordinator. The Asthma Coordinator works with the child's health care provider to coordinate care centered on a written asthma action plan. The Asthma Coordinator visits the family in their home and provides asthma education. Included is an in-home environmental assessment that assists the family in identifying and minimizing asthma triggers in the home. The Asthma Coordinator also interacts with the child's health care provider and childcare provider. Community Education
Asthma education workshops are conducted for day care providers, family day care providers, and parent groups. Childcare providers are also given information about creating "asthma-friendly" environments to help the children they serve who have asthma and allergies. Click here for a self-study module. Continuing education workshops are conducted for health care professionals wanting to learn more about asthma management following national guidelines. Included are hands-on demonstrations for using asthma medical devices and tips. As of the summer of 2006, services are provided in National City , North County and rural areas. Children who are found not to be eligible for any health insurance source, may have access to asthma treatment services through a local community clinic including medical visits, medication and medication administration devices.
